We have resolved this issue with the maintenance on 2/1/2010. You should now always receive the reward for completing missions.
Unfortunately, due to the nature of this bug we are unable to return any items that were lost.
Thank you for your understanding and for playing Star Trek Online."
I'm pretty bloody outraged. Lucky they fixed it just in time for launch. Unluckily for me, I wasted my head start because now I have to re-roll to get my officers. I feel especially bad for those who lost blue/purple ones.
