I don't know if this was ever fixed, since avoiding it became second nature to me during beta, but I used to find that if my character turned to track a target, the controls got all messed up.
The problem was that the 'paper doll' that the server sees did not turn along with the avatar that we see on the screen; so the controls reflected a different orientation than the one displayed.
In other words, if your character turned through 180 degrees because its target had moved behind it, then the controls would be reversed; this could be avoided by using the right mouse button to readjust the camera, thus turning the 'paper doll' to face the same way as the on screen avatar.
Like I say, they might have fixed that; but it may help you.
