They appear as regular popups that will show when set to triggers. They will show immediately on entering a map if not set to triggers.
I hope that helps. It is not intuitive, but they are useful once you start messing with them.
They are map wide, so it doesn't matter where you put the blue talk icons. But make sure you rename them to signify what they say, or else you might get confused with multiple dialog objects on a map.
