"Login failed for unknown reason" error
Missing additional character slot for current/ lapsed Gold Members
You have to spend all your skill points to get promoted. Then you will be promoted to Commander.
As an aside, there was one person who posted on the forums saying that she got to Ensign 51. It meant she never spent her skillpoints, and never got promoted.
Promotions are at 11 each rank, but there's a confusing bit. You'll get to LTC 11 by gaining the required skill points, but you can only get the promotion to commander 1 by spending the required skill points.
