On more than one Exploration Mission (usually variations of the 'colony map' where you drive Klingons out) one or more of the anomalies won't trigger a scan action, so you can't collect them. Planet Mu Puppis 052-VH is one example. It's almost like the actual item is embedded in the terrain and can't be accessed as a result.
