I would think operator error, on the part of the purchaser. Personally I think the OP is looking for attention. Dang nabit I just gave him some.
I don't think operator error, but I do think the error wasn't on Cryptic's end the way it sounds. The address error sounds like a bank error - if the billing address doesn't match the card's they'll return an error, and will decline payment. Issue is, by the sounds of things they were still returning funds, even though the error code prevented the account system from associating the payment with the transaction. Under no condition should a bank return both payment and an error, most billing systems have no way of handling it when it does. With the one we have at work I'm not even sure we'd be able to figure out where the money came from without the ank's help.
