So the Intrepid only violates the rules 50% of the time? And when they are raised they break the visibility from front rule, no?

And so does the Excelsior. Really the worst offender. and the refit got impulse engines in front of the nacelles...
Saber class is shady. Does its nacelles have 50% line of sight between each other?
Promethus is another one. (the lower nacelles)
Defiant. Does it even have a saucer so to speak? And nacelles are probably not looking at eachother much either.
The Raven. Well it is a starship. Nacelles wont be waving at eachother any time soon.

And the Negh'var is debatable at best. Only it's lower half of the nacelles seem to have los.
It's such a split up nacelle.
Just to add to the list, the sovy in the game has its nacelles leveled with the saucer when it is clearly in movies, shows etc the nacelles are higher. I have already mentoned this ont he starship whatever error thread but none of the devs have said anything.

As for the excelsior refit, i never understood why they added the two extra impulse engines where it can burn the front part of the nacelles.

IIRC, those were originally intended to be shuttle bays, but I think someone looking over the model said 'MORE FX!' and they changed them to impulse engines.
