Just bought an Ultimate card today and I can't use it because the checkout page only asks for a promo code and not a pin code which is what is on the back of the card. This also appears to be affecting every card other than the PWE prepaid card.
(Tried to contact support but all I get is a failed redirect)
Working on my end, I might have a idea of what you are doing ...
You select Ultimate Game Card and then it go to checkout, what happens next is you pick the amount and then click the checkout button, you are then redirected (well in this case it opens a a small window) to the site were you input the PIN number.
