Issue occurred 11/21/2012
STO developers released a patch with out proper testing resulting in wide spread disconnects and a broken reputation system.
Steps to reproduce this issue:
1) have developers release an update, it will most likely break something.
To resolve the issue there are a few approaches
1) Test patches thoroughly before pushing to production servers. This will reduce the number of bugs that are introduced to live servers.
2) Replace the development team with professionals. Skilled developers will reduce the number of bugs that are introduced to live servers.
3) If 1 and 2 fail, sell the game to a company that will properly maintain the game.
