I suspect that any Really twisted holodeck programs get flagged for Riker to trade to the Ferengi next time he plays poker

To be fair, as the CMO, he deserved to be carrying an appropriate rank, but then again, the CMO outranks the commanding officer on medical matters, so maybe a rank isn't necessary...
