According to the Lifetime Rewards section, I'm supposed to get the following (along with others) when I purchase a LTS:

• The Captain's Table - A special area restricted to lifetime subscribers and longtime STO veterans.
• "Career Officer" title - Show off that you're committed to Starfleet or the KDF for the duration.
• Forum goodies - Change the color of your username and show off a special title.

I got everything else on the list, but

1. Where/How do I access the Captain's table?
2. Why is my title not shown as "Career Officer" but "Junior Member" in the forums?
3. Forum Goodies - I can't find any of those in the My Account section on the forum

Or is it one of those items where the website is out of date and none of that stuff is available as advertised?

Yup.

Captains Table
You need to collect some free Access codes from the C-Store. Then you can then fly toward the blue orb at ESD and transport to the Captains Table.

"Career Officer" title
This is a free C-Store purchase again.
