After you complete Afterlife I figured i'd play thru pick up Gates of Grethor and then turn them in at the end. Well I though wrong the gates of grethor dropped after i went to sector space and only had afterlife to turn in. Also in Skirmish I had this problem where the federation ships would start respawning at the end so as soon as you killed one fed ship 2 more would respawn was very frustrating to be able to beam over to axon to complete the mission.

I have had that problem with the Gates of Grethor too. In fact, the second time when I killed Molar, I dismissed his dying dialog and ended up advancing to the next stage, but to a completely empty base and had to restart the mission for the third time.

Sigh.

Yeah I was not thrilled about the rewards for tribble testing for 51 but at last minute I said what the heck just incase i become an accolade hunter one day. Another one like this is that dividian mission with the flashlight. I know one reason it starts at 21 for the kdf now is because there were a ton of bugs that it bypassed having to fix for the kdf by removing that part out. All in all though if you are very fast if you don't hit any bugs you can get cold comfort, spin the wheel, and all 3 of those missions with kahless done in around 30 mins if yer lucky not to have the bug. I did notice the one bug everyone liked to level up with empire defenses is now fixed... seems like only the helpful bugs ever get fixed.
