"Login failed for unknown reason" error
Missing additional character slot for current/ lapsed Gold Members
Cold Case - the Klingon level 49 mission is bugged.
I have all the corners lit up + center, tried the wiki. No Combination works, the first square cannot be lit up successfully after the first attempt fails. Reason: No corner can be selected without moving off of the center for the PC.
Sure some people might be able to do it depending on how sensitive their controls are set up, but I cannot, perhaps because of my keyboard being a few years old.
Solution
Reduce the corner distance to the center for those of us with less sensitive controls.
The mission is not bugged, you're trying to do it wrong. The advice in the wiki is not great and harder to pull off than the 'offical' way of doing it.
