While entirely true, it's not exactly uncommon for a government/military spinning off from another government/military (such as say the American Colonial Army from the British Army as one example) to use a similar organizational and rank structure.

Unfortunately Commodore has not been canon since Kirk's time - that Rank was removed and replaces by the upper/lower Rear Admirals in TNG canon. Thus STO is actually correct with the TNG-era's Ranks.
STO is about my Liberated Borg Federation Captain with his Breen 1st Officer, Jem'Hadar Tactical Officer, Liberated Borg Engineering Officer, Android Ops Officer, Photonic Science Officer, Gorn Science Officer, and Reman Medical Officer jumping into their Jem'Hadar Carrier and flying off to do missions for the new Romulan Empire. But for some players allowing a T5 Connie to be used breaks the canon in the game.

You say it like Cryptic hasn't radically changed anything in the Trek storyline before. If a bunch of Ferengi merchants can apparently sell an endless supply of mega-warships to anyone (including individual starfleet officers), than Starfleet can un-fubar a pair of rank names.

The Commodore issue was a huge debate 3 years ago. And CBS does get the final decision on these types of things. Cryptic can't just add Commodores simply because we want them. CBS has to say OK.

I don't see why we need a lieutenant rank for Romulans.

It feels more Romulan to go like this:

Civilian/Uhlan
Sub-Lieutenant
Centurion
Sub-Commander
Commander
Admiral

These are the ranks commonly seen on screen, and so it makes sense to use them. It also makes them different from the Fed/Klingons without too much rank overlap.

It also seems to make sense that Flag Officers all have similar names regardless as this tends to be the case with all navies worldwide.
